Pendahuluan : Extracorporeal Shock Wave Lithotripsy (ESWL) bagian metode non invasive Nephrolithiasis. Nephrolithiasis penyakit yang angka kekambuhannya sangat tinggi, angka kekambuhan mencapai 60% setelah 5 sampai 10 tahun.. Studi pendahuluan di RSUD Dr. Saiful Anwar sebanyak 16 pasien ESWL dengan posisi supine 31,25% merasakan nyeri arah sumber energi dari samping. ESWL prosedur disertai rasa nyeri, arah sumber energi tembakan sangat mempengaruhi nyeri. Tujuan: untuk mengetahui perbedaan skala nyeri pasien ESWL posisi supine arah sumber energi tembakan dari samping dan bawah. Metode: Desain penelitian observasional analitik dengan pendekatan kohort. Observasi dilakukan saat ESWL sampel sebanyak 20 responden dengan tehnik purposive sampling. Instrumen penelitian menggunakan lembar observasi Visual Analugue Scale (VAS). Hasil: Rerata skala nyeri posisi supine dengan arah sumber energi tembakan dari samping 1,5 dan bawah 1,1 ada perbedaan 0,4.Uji statistik Wilcoxon pada pulse rate 5 ( 0,564 ) , 10 (0.132) , dan 12 (0.114 ). Secara deskriptif penelitian ini ada perbedaan skala nyeri pasien Nephrolithiasis tindakan ESWL pada posisi supine dengan arah sumber energi tembakan dari samping dan bawah sedangkan secara statistik tidak ada hubungan. Kesimpulan: Setiap peningkatan pulse rate perlu dikaji skala nyeri, untuk keefektifan dan keberhasilan ESWL
