Tuberkulosis (TB) merupakan penyakit infeksi yang memerlukan pengobatan jangka panjang sehingga kemampuan self-management menjadi faktor penting dalam keberhasilan terapi. Rendahnya kemampuan self-management dapat menyebabkan diagnosis keperawatan manajemen kesehatan tidak efektif. Penelitian ini bertujuan mendeskripsikan penerapan Self-Management Activity pada pasien tuberkulosis dengan diagnosis tersebut di wilayah kerja Puskesmas Waingapu. Penelitian menggunakan desain studi kasus dengan pemilihan partisipan secara purposive sampling. Intervensi meliputi asesmen kesiapan pasien, edukasi kesehatan, manajemen kepatuhan pengobatan, pencegahan penularan, pemantauan mandiri menggunakan self-management logbook, dan pelibatan keluarga. Evaluasi dilakukan berdasarkan luaran Manajemen Kesehatan dalam Standar Luaran Keperawatan Indonesia (SLKI) melalui observasi, wawancara, dan penelaahan logbook. Hasil menunjukkan bahwa Self-Management Activity meningkatkan kemampuan pasien dalam mengelola kesehatan, ditandai dengan peningkatan pengetahuan, kepatuhan pengobatan, perilaku pencegahan penularan, penggunaan self-management logbook, dan keterlibatan keluarga. Intervensi ini berpotensi diterapkan sebagai upaya pemberdayaan pasien dan keluarga di pelayanan kesehatan primer.
